How do you create help for an app?

I have created several apps and up until now they have been pretty straight forward so I've never created a help menu. However I need to now. I have no idea how to go about it. Do I code it myself? Are there third party apps designed for it? Are there any free (or cheap) ones out there?

Not sure if it matters but I'm using VB 2005.

I also know that NDoc do a pretty good job for C# 1.1 but since VB.NET 2.0 is integrating XML comments, you might do a little search to see if it's possible to compile a class help.

(was it a class help or a software documentation?)
